The following groups of individuals must undergo FBI and DOJ background checks and fingerprinting by Live Scan before beginning their employment or volunteer service:
Download the appropriate form below and take it to a Live Scan location. The cost of the background check can be refunded if needed. Once a background check has been completed, the staff member or volunteer must agree in writing to LPC, requesting, obtaining, and reviewing criminal record information (CORI) on an ongoing basis for the duration of my employment/volunteer service with the organization.

The following groups of individuals must participate in Boundaries Training every 2 years required by the PC(USA) and the state of California in compliance with AB 506 in order to best care for the vulnerable among us. The following video is 36 minutes long and addresses the PCUSA Sexual Misconduct Policy and Preventing and Reporting of Child Sexual Abuse. It can be watched from home at any time. Those required to take boundary training must record when the completed watching the video on the Acknowledgment of Consent Form below.
Note - The video refers to "Elders", but applies to all of the people listed above who are considered Mandated Reporters per California law.

Signed Acknowledgement of Consent of the above documents, as well as consent to undergo a background check and completion of boundaries training if require, is needed annually from those listed above.
